Liability Release For HeART Mending Team

I acknowledge that HeART Mending team members have voluntarily agreed to pray for me using various inner healing methods.

I understand that this session is not a professional counseling meeting and that none of the team members are licensed counselors.

I understand that these team members are, to the best of their ability, doing what they can to help me achieve more freedom in my life.

I further state that I have voluntarily sought assistance of my own initiative and that I am under no obligation to accept or reject any of the advice or help that I might receive from the HeART Mending team members.

I understand that if I participate in a HeART Mending session the team is committed to respect the disclosed information.

We are required by law to report two issues:

  1. Any intent of a person to take harmful, dangerous, or criminal action against another person or him/herself
  2. Any act of child or elderly abuse or neglect


I agree to hold all HeART Mending team members free from any and all liability, loss or damage of any kind that may arise as a result of assistance I have received or from my participation in HeART Mending sessions.

I have read this disclaimer and release of liability and understand and agree with it and have executed it as my free and voluntary act

Cancellation, Refund, No Show Policy

This time slot has been reserved for you and we trust that you will do your best to honor it by being there on time.

However, we understand that emergencies happen.  Please read our Cancellation, Refund, No Show Policy.



There will be no charge for a cancellation submitted at least 48 hours in advance of a HeART Mending session. This allows time for someone else to request that session.  Less than 48 hour cancellation results in no refund unless there are extreme circumstances, which must be in writing and emailed for consideration to the HeART Mending team at: [email protected]

No show at the time of a HeART Mending session results in no refund unless there are extreme circumstances, which must be in writing and emailed for consideration to the HeART Mending team at:  [email protected]

More than two cancellations may result in not being permitted to participate in future HeART Mending sessions.
